Patient Rights Include:

- Getting a digital paper copy of your electronic medical record as filed.
- Correcting our copy of your paper or electronic medical record.
- Asking us to limit the information we share.
- Getting a list of individuals/companies with whom we have shared your information.
- Choosing someone to act on your behalf.
- Getting a copy of these patient rights.
- Filing a complaint if you believe your patient rights have been violated.
Patient Choices Regarding How We Use and Share Information:
- Share your condition with family and friends.
- Include your name in a hospital directory.
- Market our services and sell your information.
We May Use and Share Your Information When We:
- Care for you.
- Operate our business.
- Bill for your services.
- Assist in public health and safety issues.
- Comply with the law.
- Respond to workers’ compensation, law enforcement, and government requests.
- Help with organ and tissue donation requests.
- Work with a medical examiner or funeral director.
- Conduct or direct research.
- Assist in responding to litigation issues.
Our Responsibilities Include:
- Maintaining the privacy and security of your protected health information.
- Letting you know promptly if a breach occurs that may have compromised the privacy or security of your information in our files.
