Patient Rights and Information

Patient Rights Include:

• Requesting confidential communication with our office.

• Getting a digital paper copy of your electronic medical record as filed.

• Correcting our copy of your paper or electronic medical record.

• Asking us to limit the information we share.

• Getting a list of individuals/companies with whom we have shared your information.

• Choosing someone to act on your behalf.

• Getting a copy of these patient rights.

• Filing a complaint if you believe your patient rights have been violated.

Patient Choices Regarding How We Use and Share Information:

• Share your condition with family and friends.

• Include your name in a hospital directory.

• Market our services and sell your information.

We May Use and Share Your Information When We:

• Care for you.

• Operate our business.

• Bill for your services.

• Assist in public health and safety issues.

• Comply with the law.

• Respond to workers’ compensation, law enforcement, and government requests.

• Help with organ and tissue donation requests.

• Work with a medical examiner or funeral director.

• Conduct or direct research.

• Assist in responding to litigation issues.

Our Responsibilities Include:

• Maintaining the privacy and security of your protected health information.

• Letting you know promptly if a breach occurs that may have compromised the privacy or security of your information in our files.

• Following the duties and privacy practices described in this HIPAA notice and provide a copy to you upon request.

• Using or sharing your information, other than as described herein, unless you request a modification in writing. If you make such a request, you can  change your wishes at any time. You must let us know in writing if you change your mind regarding the use of your information.

• We may be required to share your information in other ways, usually in ways that contribute to the public good, such as public health and research.

• Meeting several conditions in law may apply before we are allowed to share your information for these purposes.

Changes to the Terms of this Notice

We may change the terms of this notice, and the changes will apply to all data we have about you. The new notice will be available on our website.

This notice of privacy applies to Texas Psychiatry Group PLLC including authorized clinical staff, medical physicians, and payors.
